Authorization for Treatment

I hereby give permission to the medical personnel selected by the RHOM director to administer medications; to order treatment; to release any records necessary for insurance purposes; and to provide or arrange necessary related transportation for me/or my child. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the RHOM director to secure and administer treatment, including hospitalization, for the person named above. I agree to remain fully liable and responsible for any such incident and fees.

 

Communication & Privacy

In the event of time-sensitive class schedule changes or cancellations, we may attempt to contact you by email or phone call. By providing a telephone number during registration you are consenting to be contacted by phone for customer service purposes. Message & data rates may apply.

No personally identifiable information will be shared with third parties or affiliates for marketing or promotional purposes.

 

Client Cancellations and Missed Classes

Group Classes: Due to the collaborative nature of group classes, we cannot issue refunds or credits for classes missed by clients. Missing classes DOES NOT disqualify you from the class. Please join us again as soon as your schedule or health permits.

Private Lessons: We will reserve your weekly lesson time through the end of the semester. If you cannot make it to your scheduled lesson, your teacher will send you a message and video outlining a skill or song to practice. We cannot issue refunds or credits for lessons missed by clients.

 

Teacher Cancellations

Group Classes: If a class is canceled due to teacher absence, we will attempt to find a substitute. If a substitute is not available, we will issue a credit to your account.

Private Lessons: If a lesson is canceled due to teacher absence or weather, we will attempt to reschedule the lesson. If the lesson can't be rescheduled we will issue a credit to your account.

 

Withdrawal & Refunds

We cannot issue refunds to students who decide to withdraw, but our staff is here to help problem solve and find the right placement for you. If you have concerns about your experience at Rocktown House of Music, please contact director@rocktownhouseofmusic.com

 

Photo & Video Permissions

I grant Rocktown House of Music permission to photograph or video my/my child's participation in Rocktown House of Music education programs. I also grant Rocktown House of Music permission to use my/child's likeness in a promotional capacity including but not limited to print materials, web site, print, online or out of home media and other medium now or later developed; as well as permission to share photography and video including my/my child's likeness for use by media publications and institutions that fund Rocktown House of Music operations and programs.
